
EARLY INTERVENTION:
Immediate three point contact with injured worker, medical
provider, and employer to assess appropriateness of medical
treatment and clarify work status. Once the three point
contact is completed within 24 - 48 hours of referral, the
information obtained and recommendations of the case manager
are provided to the referring source immediately.
TELEPHONIC CASE MANAGEMENT (TCM):
The Nurse Case Manager establishes and maintains phone
contact with the injured worker, medical provider, and
employer to promote proper medical treatment and early
return to work. TCM is most effective immediately after an
injury and is usually completed in 30 day increments.
MEDICAL CASE MANAGEMENT:
The Nurse Case Manager establishes and maintains face to
face contact with the injured worker, medical provider and
employer. The Nurse Case Manager coordinates medical
services and coordinates a return to work as soon as
medically appropriate.
VOCATIONAL REHABILITATION:
An injured employee who can not return to their usual and
customary employment, may be entitled to vocational
rehabilitation benefits. The Vocational Counselor provides
vocational assessments to determine the employee's aptitude
and capabilities for job placement or retraining. The
Vocational Counselor will also assist and counsel the
employee on job search activities, interviewing activities,
resume building and vocational training.
